Uniform Dental Benefits

If you have medical coverage under the State of Wisconsin Group Health Insurance Program, you have the option to elect the Uniform Dental benefit. Employees choose health insurance, with or without dental, during the enrollment process.
The Uniform Dental benefit is onlyavailable if you enroll in the State Group Health Insurance program. Supplemental dental insurance (Dental Wisconsin and EPIC Benefits+) are separate, optional plan offerings.
If you elect family health insurance coveragewith dental , you will be enrolled in family dental coverage. Similarly, if you elect single medical coveragewith dental , you will be enrolled in single dental coverage.
Delta Dental of Wisconsin is the plan administrator of the Uniform Dental benefit.
Below is the schedule of benefits covered under the Uniform Dental benefit. You must use a Delta Dental PPO or Premier provider to receive benefits.
Uniform Dental provides coverage for diagnostic, preventive and restorative services (such as fillings). It does not include coverage for major dental services, such as crowns, root canals or implants. If you want coverage for major dental services, you may want to consider enrolling in Dental Wisconsin, EPIC Benefits+ or both.

Dental expenses covered under the Uniform Dental benefit are not subject to the health insurance deductible.

Delta Dental has two provider networks available under the Uniform Dental benefit: Delta Dental PPO and Delta Dental Premier. To receive coverage for dental services, members must choose a dentist within one of Delta Dental’s dentist networks. There is no benefit for out-of-network providers.

The dental premium will be included in your health insurance premium. You will not see a separate deduction on your paycheck for Uniform Dental benefit premiums. For State Group Health premiums, with or without Uniform Dental benefits, see the Premiums page.
